visual studio 2010 - Auto-populating new panel on button click -

and thank viewing thread. have project creating in visual studio 2010 c#. word minutes template. have form user fills out, goes word document. have gotten stuck problem. try best explain. have section user keeps entering data on subject. need able have form continuously populate same panel until user clicks check box moves user next question. right now, have panels on top of each other , have them hide until user checks checkmark , moves on. problem have no idea how many possible panels needed. user have 5, or user have 100 if needed. there easier way have form populate panels automatically? attach screenshot below. help!
